Treatment Overview

Glaucoma is a group of eye diseases that can damage the optic nerve, often due to high intraocular pressure (IOP). If left untreated, it can lead to permanent vision loss. When medications and laser treatments fail to control IOP, glaucoma surgery becomes an essential option to prevent further damage and preserve vision. The primary goal of glaucoma surgery is to reduce intraocular pressure by improving fluid drainage from the eye or by creating a new pathway for fluid to flow.

There are several types of glaucoma surgeries, each tailored to the specific needs of the patient. The most common procedures include trabeculectomy, tube shunt surgery, and minimally invasive glaucoma surgery (MIGS). Trabeculectomy involves creating a small hole in the sclera (the white part of the eye) to allow excess fluid to drain, thus lowering IOP. Tube shunt surgery uses a tiny tube to direct the fluid from the eye into a drainage area. MIGS techniques, on the other hand, use small incisions and microdevices to improve fluid drainage with less risk and quicker recovery times.

The choice of surgery depends on various factors, including the type and severity of glaucoma, the patient’s overall eye health, and their response to other treatments. For example, in patients with severe glaucoma or those who have not responded to other treatments, tube shunt surgery may be recommended. For those with mild to moderate glaucoma, MIGS procedures may provide effective results with fewer complications. In all cases, the surgery aims to lower IOP and prevent further optic nerve damage, thereby maintaining the patient's vision for the long term.

Post-surgery recovery generally involves monitoring IOP and avoiding strenuous activities that may increase pressure in the eye. While glaucoma surgery can significantly reduce the risk of blindness, it does not cure the disease. Regular follow-up visits to an ophthalmologist are crucial to ensure the IOP remains stable and to detect any signs of complications. With timely intervention and appropriate surgical management, most patients can maintain a good quality of life and preserve their vision.
